Information we collect
You may always choose not to share your Personal Information with us. However, certain Personal Information is required to file and process a claim. Failure to provide required information may result in us not being able to file or process your claim. For additional information about what Personal Information is required to file and process a claim, please contact privacy@coterieinsurance.com.
Additionally, please only provide the information that we request relevant to your claim. If you provide additional information that we have not requested, that information will be considered to be submitted to us with your consent and used to process your claim.
During the claims process, we may collect and use the following Personal Information and Sensitive Personal Information. Note, some Personal Information included in each category may overlap with other categories.
Identifiers, such as real name, postal address, unique personal identifier, online identifier, Internet Protocol address, email address, Social Security number, driver’s license number or state identification card number, or other similar identifiers.
Other Claimant Personal information, such as signature, physical characteristics or description, insurance policy number, Medicare Policy Number, employment, medical information, or health insurance information.
Protected classification characteristics, such as age (40 years or older), Pregnancy, medical condition, or physical or mental disability.
Internet or other similar network activity, such as browsing history, search history, information on your interaction with our website, application, or advertisement.
Geolocation data, such as imprecise physical location when you use our website or application.
Professional or employment-related information, such as current or past job history
Sensory Data, such as recorded telephone calls or virtual meetings.
Sensitive Personal Information, such as Social Security number, state identification card Number, or driver’s license number; Claimant health information; contents of a claimant’s mail, email, and text messages when relevant to a claim. Note that we do not collect Sensitive Personal Information for the purpose of making inferences about you.
Use and disclosure of information
We may use and disclose Personal Information for the following purposes:
- Auditing related to counting ad impressions to unique visitors, verifying positioning and quality of ad impressions, and auditing compliance with this specification and other standards. Note, we do not disclose medical-record information, privileged information, or Personal Information relating to an individual’s character, personal habits, mode of living, or general reputation, nor any classification derived from the information for this purpose.
- Helping to ensure security and integrity of our Services, including detecting or preventing criminal activity, fraud, material misrepresentation, or material nondisclosure in connection with an insurance transaction.
- Debugging to identify and repair errors that impair existing intended functionality.
- Short-term, transient use, including, but not limited to, non-personalized advertising shown as part of an individual’s interaction with us.

Note, we do not disclose medical-record information, privileged information, or Personal Information relating to an individual’s character, personal habits, mode of living, or general reputation, nor any classification derived from the information for this purpose.

- Undertaking activities to verify or maintain the quality of our Services, and to improve, upgrade, or enhance our Services.
- With your authorization, consistent with appliable law.
- Determining an individual’s eligibility for an insurance benefit or payment.
- Verifying insurance coverage or benefits.
- Informing an individual of a medical problem of which the individual may not be aware.
- Conducting operations or services audits.
- To a party or a representative of a party to a proposed or consummated sale, transfer, merger, or consolidation of all or part of our business.
- To a governmental authority for the purpose of determining the individual’s eligibility for health benefits for which the governmental authority may be liable.
- To a certificate holder or policyholder for the purpose of providing information regarding the status of an insurance transaction.
- To a lienholder, mortgagee, assignee, lessor, or other person shown on the records of an insurance institution or agent as having a legal or beneficial interest in a policy of insurance.
- To an insured or the insured’s lawyer when the information disclosed is from an accident report, supplemental report, investigative report, or the actual report from a government agency or is a copy of an accident report or other report that the insured is entitled to obtain under applicable law.
- To an insurance regulatory authority.

Retention
We will retain and use your Personal information and Sensitive Personal Information for the period necessary for the specified purposes for which we collect it and to comply with our legal obligations, unless a longer retention period is required or permitted by law.
